UK Medical Card

Once you have purchased your medical card you will receive and email asking for the following information: 

1. The name of the Company and Pharmacy you use. For example: Alternaleaf / Montu
 
2. A Photo of your latest Prescription

3. A clear headshot photograph of you, D.O.B, Full name and address.
